You may need to submit dependent verification documents and/or evidence of insurability for some changes. Changes are effective the first day of the month after you notify Benefits Outlook. For example, if you get married June 21 and notify Benefits Outlook July 5, your changes become effective August 1.

Birth or adoption

Within 31 days of your child's birth, contact Benefits Outlook to change coverage for you, your spouse and/or your dependent(s). Dependent reaches age 26

When your dependent child or your dependent grandchild turns 26, they’re no longer eligible for benefits unless they’re disabled. Your child’s benefits are automatically terminated on the last day of the month they turn 26. You will receive an email notification of the termination.

  

Once your child is no longer covered under your benefits, they’re eligible to receive coverage through COBRA if they enroll within 60 days of their birthday.

 

If your adult child is disabled, contact Aetna customer service at 877-224-6857 to fill out the Disabled Dependent application. Because this will have to go through Aetna approval process, this will need to be initiated PRIOR to the disabled dependent’s 26th birthday.

 

A note about cancer care coverage

Coverage for your child under this plan ends at age 19 unless your child is an unmarried full-time student. In that case, this coverage is available through age 25.

Divorce or annulment

Qualified medical child support order

A qualified medical child support order (QMCSO) is a court order requiring you to add medical coverage for your child. Here’s how it works:

  • The court order goes directly to the Risk Management office for processing.

  • If you already have medical coverage through the district, your child will be enrolled in the same plan.

  • If you’re not enrolled, you’ll both be enrolled in the Memorial Hermann ACO plan. Look for a letter or email notification.

  • The effective date of the change is the first day of the month following receipt of the order.

 

Risk Management also receives and processes National Medical Support termination notices. When they receive a termination order, they’ll notify you of the option to keep or drop the coverage for your child. Any changes become effective the first day of the month after they receive your election change.
