Follow these instructions for Davis Vision registration help.

What's included in the vision plan?

The vision plan offers in-network and out-of-network care. That means you can use any vision care provider you want and receive benefits. Your cost is lower when you use in-network providers. Aflac Vision offers array of independent and retail providers. If you choose to receive treatment or services from providers outside the network, your share of the cost is higher and you’re responsible for expenses that exceed the schedule of benefits. If you select vision coverage, you and your enrolled dependents can each visit an optometrist or an ophthalmologist for one eye exam and one pair of eyeglasses or contacts once every calendar year.

Does the vision plan cover contact lenses?

Yes. If you select vision coverage, you and your enrolled dependents can each receive contact lenses benefits in lieu of eyeglasses once a calendar year.  The in-network allowance for contact lenses is $130 with a 15% discount on amount exceeding the allowance. 

How do I choose a provider?

On the Aflac Vision portal (Davis Vision Network), please log in and click the Search for Providers option. The Provider Finder will display a full list of all conveniently located providers participating in your plan. You can also call Aflac's KISD dedicated Vision line (Davis Vision) at 800-999-5431 for assistance in locating a provider.

How are vision benefits paid?

If you choose an in-network provider:

  1. Make an appointment with the in-network provider chosen.
  2. Identify yourself as an Aflac Vision participant and give the provider your ID number off your ID Card and the patient's date of birth to verify coverage. If you do not have your ID number, the provider can verify off your/patient’s name, date of birth and home address.
  3. If you are fitted for eyeglasses, you may purchase them during this visit or go to another network provider. You have a $130 allowance towards any frame OR Up to$180 allowance (at Visionworks loactions only). You also have some covered-in-full lens options, which are polycarbonate lenses, standard scratch-resistant coating and solid/gradient tints. Should you choose options that are not covered within your plan allowance, simply pay the difference at the time of service.

That's it. You don’t need to file a claim when you receive care from network providers.

If you choose an out-of-network provider:

  1. Make an appointment with any licensed provider.
  2. Pay the full cost at the appointment.
  3. Submit a Davis Vision Claim Form with your itemized receipts and member’s Aflac Vision ID number and patient's date of birth to Aflac Vision (Davis Vision) and mail it to the address below.

      4. Receive reimbursement according to the schedule of benefits.

How do I get an ID card?

If you need vision care services and your Aflac Vision ID Card has not yet arrived in the mail, you can print off a temporary ID card. Simply log in as a member, select "Print ID Card," and print! If you are looking for a hardcopy ID card, please call Aflac's KISD dedicated Vision line (Davis Vision) at 800-999-5431.

What are medically necessary contacts?

When a vision care provider has determined that spectacle lenses and frames will not achieve the best vision correction possible due to a medical condition, those contact lenses are deemed medically necessary. In most cases, the following diagnoses will qualify a member for medically necessary contacts (prior approval is required):

• Keratoconus

• Corneal trauma

• Post-cataract surgery

• Anisometropia

If I need medical or surgical treatments for eye disease or any eye injury, should I go to an Aflac Vision (Davis Vision Network) provider?

No. Your major medical plan will usually cover eye disease or injury. The Aflac Vision plan covers primary vision care. If you need help locating an appropriate medical vision provider, please contact your health plan's customer service department at 800-999-5431 Monday – Friday: 8AM – 11PM (ET), Saturday: 9AM – 4PM (ET), Sunday: 12PM – 4PM (ET).

If you have a medical emergency, call 911 or go to the nearest hospital emergency room.
