What coverage does Katy ISD provide?

What are the enrollment rules for Supplemental Life benefit?
  • If you are newly benefits-eligible, you may purchase supplemental life insurance coverage in increments of $10,000 up to $300,000 without evidence of insurability (EOI).

  • Current employees enrolling for the first time can enroll for $10,000 without providing evidence of insurability (EOI).  Additional coverage may be added pending EOI approval.
What are the options for child life/spouse life coverage?

You may also buy life insurance coverage for your spouse and your children. 

If your spouse also works for Katy ISD, you can both have supplemental life coverage, but neither can enroll for spouse life coverage. A child may not be insured by more than one employee.

You have the following coverage options for your spouse:

  • $10,000
  • $20,000
  • $35,000

Your spouse life coverage must be less than or equal to 50% of your combined basic and supplemental coverage. For example, if your combined basic and supplemental life and AD&D coverage is $50,000, the most coverage your spouse can have is $20,000.

You have the following coverage options per child:

  • $5,000
  • $10,000
Are there age limitations for the spouse and child(ren) life coverage?

Yes, children over the age of 26 are not eligible. Eligibility is determined when a file is claimed, so it’s your responsibility to be sure your child(ren) meet the eligibility requirements when enrolling. There is no age limitation on spouse life insurance coverage, the rates are based on spouse age.

What is evidence of insurability (EOI)?

EOI verifies your general health for underwriting purposes and provides the insurance company with information to determine if you can enroll. If the provider requires EOI, you simply complete and submit your form through OneAmerica's company website or mail back the EOI form you received.

Coverage that requires EOI is effective the first of the month after the company approves your EOI. If you sign up during benefits enrollment, your coverage can’t start before the new plan year begins.

Do I need evidence of insurability (EOI) to enroll for coverage?

EOI provides the insurance company with information in order to be considered for certain types of insurance coverage.

You do not need to provide EOI to:

  • Enroll in supplemental life coverage when first eligible up to the maximum amount of $300,000.
  • Increase current coverage by 1x your annual base salary (in increments of $10,000) during benefits enrollment if previously enrolled.
  • Elect $10,000 during annual enrollment if not previously enrolled.

You must provide EOI to:

  • Enroll in greater than $10,000 if not previously enrolled.
  • Increase current coverage by more than 1x your annual base salary during benefits enrollment.
  • Increase spouse life coverage to more than the next level. You can only move one level up each annual enrollment.

If EOI is required, you are alerted during the enrollment process and OneAmerica sends the appropriate forms to your home address. To finish the enrollment process, you must complete, sign and return the EOI form by the date specified. The form also contains instructions for completing the process online. New coverage becomes effective the first day of the month following the date when the required form and any additional information requested is approved by OneAmerica. If you sign up during benefits enrollment, your coverage cannot start before the new plan year begins.

How do I designate a beneficiary?

You may designate a beneficiary online during enrollment or anytime during the year.  You may also call Benefits Outlook at 866-222-KISD(5473) and speak to a customer service representative. Your beneficiary designation applies to both the district-provided basic life insurance and the supplemental life, if enrolled. You are automatically the beneficiary for the spouse life or child life, if enrolled.
